- How to complete the form:

1) Open the PDF document (you need Adobe Acrobat Reader 8.0 for this – download it here).

2) Complete the form electronically. To enter the prices in the relevant boxes, see Fees 2009 (pdf).

3) Once completed, print the form by clicking on the button at the top right-hand side of the second page.

4) Then click on “Send form by email” (also at the top right-hand side of the second page) and follow the program's instructions to select the relevant application: Outlook, Yahoo, Hotmail, etc. Sending the file online pre-registers you for the course.

5) Sign the second page of the form in the relevant places (two signatures in total) and send this page by fax to +34 934 178 768. Alternatively, scan the page with the signatures and send it by email to galaint@galainternacional.com.

- Comment remplir le formulaire:

1) Ouvrir le document PDF (Décharger ici Adobe Acrobat Reader 8.0).

2) Remplir le formulaire directement. Pour les tarifs remplir les cases correspondantes, consulter Tarifs 2009 (pdf).

3) Une fois le formulaire rempli, imprimer le dossier en cliquant sur la partie supérieure à droite de la deuxième page.

4) Ensuite, cliquer sur “envoyer le formulaire par e-mail” (toujours partie supérieure à droite de la deuxième page) et suivre les instructions du programme en sélectionnant le menu: Outlook, Yahoo, Hotmail etc. L’envoi du dossier online servira de réservation optionnelle du cours.

5) Signer la deuxième page sur la partie indiquée (deux signatures au total) et envoyer cette page par fax: 34 934 178 768, ou bien scanner la page signée et l’envoyer par e-mail à l’adresse : galaint@galainternacional.com.
